Position Summary:

This position will assess the needs of patients and implement cost effective alternative plans of care to positively impact the quality of life. This position will also provide education to patients who are in need of disease management and utilize an interdisciplinary team approach when developing interventions for members and report quality trends and metrics.

Job Responsibilities:

  • Identifies and engages patients with a focus on improving patient experience, improving health and reducing costs.
  • Organizes patient care activities to facilitate optimal delivery of health care services.
  • Organizes care to involve a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs.
  • Assists in patient education regarding disease process and creates patient centric interventions to overcome barriers.
  • Educates and creates tools to encourage patients to comply with physician care, medication compliance, and decrease utilization.
  • Collaborates with hospital case management to facilitate safe discharge.
  • Assists in facilitating transition of care from inpatient status to home.
  • Completes assessments of high risk patients to prevent readmissions.
  • Maintains current knowledge of health plan benefits and provider network.
  • Evaluates patients to verify all quality improvement measures have been completed through case review and follow up as needed.
  • Nurtures effective working relations with physicians and staff.
  • Other duties as assigned.

Education and/or Experience

  • Current LVN license must be active for the State of Texas.
  • 1 year experience in disease management
  • Case management and discharge planning experience is preferred
  • Knowledge of medical terminology required
  • Strong computer skills, including MS Word and Excel
  • Strong oral and written communication
  • Strong interpersonal and organizational skills
